Directions: Please print this page and put an "X" by an meet that your swimmer(s) CANNOT make. If you can make the meet, put nothing. Unless notified we will assume that swimmers(s) will be available for all the meets.

If you have more than four swimmers please add the extra at the bottom or print a new form. We need to know if your swimmer will not be at a meet by Wednesday before the meet, so the coaches can make their line ups and plan the relay race effectively.
